Questionnaire Analytics
You send the same outcome measure or satisfaction survey to patient after patient. Someone completes it, you review their score, you move on. But at the end of a busy month, you have no real sense of how your caseload, as a whole, is doing. Are most patients completing it? Which questions are patients consistently rating high or low? Is the pattern you are seeing with one patient showing up across your whole practice?
To answer those questions, you would have had to open every patient profile individually. Most practitioners don't do that, not because they don't care, but because there was no efficient way to.
Questionnaire Analytics gives you that view without the manual work.
What it means for your practice
Instead of a series of isolated data points, you get a full picture. You can see where patients are struggling consistently. You can identify which questionnaires have low completion rates and follow up before they become gaps in the record.
It's the difference between knowing how one patient is doing and knowing how your practice is doing.
If you are a clinic manager, you have the option to look at one practitioner at a time, rather than trying to make sense of the whole clinic at once.
What you can see
The Questionnaire Analytics page for any questionnaire shows three summary figures at a glance:
- Total count: How many times the questionnaire has been shared
- Completed count: How many times it was fully completed
- Completion rate: The percentage of questionnaire shares that were completed
Below those figures, each question is listed individually with the number of patients who selected each possible response. Simple aggregate data across your entire patient population for that form.
Clicking View responses on any question shows a detailed breakdown: every patient who answered, the date they responded, and the specific answer they gave. From there, you can click through directly to any individual patient record.
How to access questionnaire analytics
- Go to HEP in the main navigation and select Questionnaires
- Click on the questionnaire you want to review
- Scroll to the bottom of the questionnaire page and click Analytics

Viewing individual patient responses
If you want to review questionnaire history for a specific patient, that is available directly from their profile. Under Progress, select Questionnaires to see everything shared with that patient — completion status, scores, and a score history chart showing trends over their course of treatment.

Availability: AI DocBuilder is available onTier 3 Practice Management memberships. Your first 50 pages are included at no cost — after that, additional pages are $0.10 per page.
How to use AI DocBuilder
It's simple. Go to HEP > Questionnaires and click Add questionnaire as you normally would. Then, select a PDF on your device.
Similarly, for chart items, go to Charting > Chart settings > Chart items and click New chart item. Then, select a PDF on your device.
