Creating questionnaire groups
If you often send multiple questionnaires to your patient at the same time or if you would like to send questionnaires when a consult is scheduled, add them to a questionnaire group so you can send them all in just a few clicks.
Go to HEP > Questionnaires > Questionnaire groups. If you have previously created any questionnaire groups they will be listed here:
Click on New group to create a new questionnaire group, give your questionnaire group a name, and click Submit (your patients will not see the name of the questionnaire group):
Note: A questionnaire must first be added to your account before you can add it to a questionnaire group. Learn how to add questionnaires to your account.
You can now add questionnaires to your group by clicking on Add questionnaire.
In the pop-up form, select the questionnaire, the Share timeline, and the Fill frequency.
The Share timeline field can be used to specify when the questionnaire should be sent to the patient. You can choose one of the following options:
- When the consult is scheduled;
- A certain number of hours before the consult;
- After the consult; or
- A certain number of hours after the consult.
The Fill frequency field can be used to specify how often the patient should fill this questionnaire. You can choose one of the following options:
- Complete for every consult: The patient will be asked to fill out this questionnaire, even if they have previously filled out a similar one.
- Skip if the patient completed this questionnaire as part of a previous consult: The patient will not be asked to fill out this questionnaire if they have previously filled out a similar one.
- Skip if the patient completed this questionnaire as part of a previous consult within: If you select this option, you will prompted to enter a duration (such as 1 year, or 1 month). The patient will not be asked to fill out this questionnaire if they have previously filled out a similar one within the duration you have entered.
The For recurring consults field can be used to specify the sending behaviour for recurring consults. This field will be disregarded if the scheduled consult is not a recurring consult. You can choose the following options:
- Send after every occurrence
- Only send after the first occurrence
- Only send after the last occurrence
Note: The Share timeline, Fill frequency, and For recurring consults behaviour will only be applied if the questionnaires are sent as part of a consult. If the questionnaire group is manually shared with a patient, all questionnaires in this group will be sent together immediately.
Repeat this step until you have added all the questionnaires you would like to this group.
